Report on the Incident at Rhode Island Manufacturing Plant on Februrary 25th, 2025
Location: Building 4 of the Rhode Island Manufacturing Plant, near the cholesterol feedstock tank.
Description of Incident: On February 25th, 2025, during routine transport of the Albany Reagent, a substantial amount was spilled into the cholesterol vat without being observed. This was not caught until substantial time had passed. Six months after there were reports of unusual symptoms in patients taking estradiol produced in the plant. Process monitoring footage confirmed the incident while Edward “Slippery” Ross was transporting a container of anhydrous Albany reagent.

Incident Documentation:
After causation was established, the system was purged and cleaned thoroughly with strong bases and detergents until reagent Albany could no longer be detected in the system. The material still inside was disposed of in the on-site disposal facility high pressure incinerator.
Externalities: The contaminated batch worked its way through a tens of people taking estradiol, causing more than expected feminization and a slight degree of overdosing that had few substantially deleterious effects given the body image altering nature of the substance.
Another problem is that EPE-Albany is highly durable and long lasting in nature. It appears to also have substantially better resistance to being denatured by ultraviolet light in nature than estradiol, bio-degradation is slow but not nonexistent. Water samples from miles downstream from centers of usage have tested positive for EPE-Albany. 5% of samples taken of locally grown food have also shown measurable levels. If no organisms are able to become more effective at breaking down EPE then this may find its way into agriculture and pose a more substantial problem.
Actions Taken:
- Implement changes in process assurance to check for the presence of EPE-A in the products and runoff.
- Develop chemical tests to ensure that it does not contaminate the environment or products.
